Compliance Checklist
Please see below the list of the document we need to register you with MSL Healthcare:

Right to work in the UK and proof of your National Insurance Number
• Application form including handbook
• Occupational Health form and associated blood reports
• CV – detailing a minimum of 10 years work/education history explaining any gaps
• 2x References
• NMC Statement of Entry
• 2 x current proof of addresses
• DBS application/DBS Update Service
• Overseas Police Check (if applicable)
• Degree Certificate
• Life Support Certificate (ALS, BLS, PBLS etc.) – No in date training? Please contact us so we can arrange a course for you
• Annual competency updates
